What we collect
When a facility sends a booking request we collect the facility name and type, the Program Director's name, email and optional phone number, a preferred day and time, and any notes you choose to add. Healers who are invited to teach give us their contact details and the documents needed to vet them.
How we use it
We use this information to review requests, match and schedule healers, send reminders and invoices, and run quarterly reports for the programs we serve. We do not sell personal information.
Patient feedback
Anonymous class feedback asks for a rating only. It collects no names and no health information, and you should not enter any.
Who sees it
Our team, and the service providers that help us run Theta State (hosting, email, text messages and payments). They may only use it to provide those services.
Your choices
